please be sure to read your step-by-step
instructions carefully
next you will need to fill out your
result notification form
thoroughly be sure to write legibly
next you will need to fill out a sample
envelope for each individual being
tested
an extra sample envelope and packet of
swabs are provided and the chance an
error was made during collection
or if you would like to add an
additional participant to the test
keep in mind there is an additional fee
to include an additional participant

before beginning remove the first swab
from the sterile packaging take care not
to touch the tip of the swab
collect cheek cells by rolling the tip
of the sample collection swab
firmly on the inside of the cheek 30
times each
or for about 30 seconds be sure to move
the swab over the entire
inner cheek surface this action should
be firm but not aggressive
and should not cause discomfort repeat
this process with the second swab on the
other cheek
use one swab for the left cheek and the
other swab for the right sheet
once both cheek swabs have been
collected place directly into sample
envelope and seal with tape
repeat this process for each participant
lay sample envelopes flat on a clean dry
surface
and let the samples dry when you have
collected all dna samples
put them inside the postage page return
envelope along with the result
notification form
and return to my forever dna please take
care to ensure
nothing is forgotten once samples arrive
at our laboratory you can expect an
email notifying you of their arrival
your results will then be emailed within
one to three business days
we will correspond with you via email
address you provided on your result
notification form in order to ensure we
treat this
as the personal and sensitive matter
